dcm4che (self-host): sovereignty score and 8 European alternatives

dcm4che is an open-source collection of applications and utilities for healthcare IT, built around the DICOM medical imaging standard. It includes a Java toolkit and library for reading, writing and manipulating DICOM files, command-line utilities for administration, data conversion and quick checks, and dcm4chee Archive 5, an IHE-compliant image manager and archive for storing, managing and retrieving DICOM images. Hospitals, imaging centres and software vendors use it to build or run archive and image-management functions. It is distributed as software that organisations install and operate themselves.

The project has community channels on GitHub and in a Google group. J4Care GmbH, based in Mödling, Austria, describes itself as currently the main developer of dcm4che and sells certified products and professional services built on it. Because the code is open source and can be self-hosted, a European organisation decides where it runs and who administers it, so jurisdiction follows the operator rather than the project. J4Care recommends its licensed enhanced solutions or professional services to bring an installation to production readiness.
