OpenMRS: sovereignty score and 8 European alternatives

OpenMRS is an open-source electronic medical record system built by a global community. It can be adapted to many clinical areas, including primary care, non-communicable diseases, maternal and child health, HIV, TB and malaria. It offers REST and FHIR APIs for integration with other systems and is easy to translate. The software is a Java web application that runs on an application server such as Tomcat or in Docker containers. There are no licence fees and no vendor lock-in.

OpenMRS is released under the Mozilla Public License 2.0 with an added Healthcare Disclaimer. OpenMRS, Inc. is a nonprofit in the United States, incorporated in Indiana, which holds the project's trademarks and assets and represents it in legal and financial matters. There is no commercial vendor with a parent company. A European organisation can install OpenMRS itself and keep all data in the EU. The project's stated mission is care in resource-constrained environments, so operation, security and GDPR compliance remain with the organisation or a service provider it chooses.

Why digital sovereignty matters with OpenMRS

OpenMRS is headquartered in EU and therefore falls under European law: the GDPR, NIS2 and the Data Act. No foreign legislation can compel access to your data.

Data is hosted exclusively in the EU, so OpenMRS scores best on data residency.

OpenMRS is open source. You can audit the source code, self-host the software and keep the encryption keys under your own control. Switching is easier because data and software are not tied to a single party.
