Nextcloud Forms is an app for Nextcloud that lets you create, share and manage simple forms and surveys. It offers checkboxes, multiple choice, dropdowns and short or long text answers. Forms can be shared by link, with specific users or with the whole instance, optionally with an expiry date. Results can be viewed as summary charts or individual entries and exported to CSV. Webhooks are supported. You install the app in one click from the Nextcloud App Store on a Nextcloud server.

The app is open source under the AGPL-3.0 licence. Forms and responses stay on your own Nextcloud instance rather than going to a third-party survey service. Nextcloud GmbH, the company behind Nextcloud, is based in Stuttgart, Germany. Where the data is stored is decided by the organisation itself: on its own servers or with a hosting provider of its choice.
