Microsoft Forms is a tool for creating surveys, quizzes and polls and seeing the results as responses come in. A form can include choice, text, rating and date questions, plus ranking, Likert, file upload and Net Promoter Score questions, grouped into sections. Forms works with Microsoft 365 work or school accounts and personal Microsoft accounts, and can be inserted into Excel workbooks and OneNote notebooks. For more advanced branding and analysis, Microsoft points to Dynamics 365 Customer Voice. It runs in the browser as a cloud service.
Microsoft is a US company headquartered in Redmond, Washington, and Forms is proprietary. For tenants whose default geography is in the EU or EFTA, the default storage location for Forms data is the EU/EFTA geography. Unless the Product Terms say otherwise, Microsoft gives Forms no specific data residency commitment, so this is a default rather than a contractual guarantee. As a US provider, Microsoft is subject to the US CLOUD Act, which lets US authorities compel production of data it controls, even when stored in Europe.
